Ph.D. Application Deadline
Acceptance of applications for admission into the Ph.D. program
in epidemiology is held once a year, with entry beginning
in the Fall semester. Applications must be received by January
31st, but for maximum consideration, applications should be
submitted as early as possible.
Ph.D. Admission Requirements
Applicants may begin the application process online at http://www.applyweb.com/aw?miamigr. Do not send any application materials to the Department of Epidemiology and Public Health. All application materials must be delivered to:

The applicant's COMPLETED file must contain the following:
1. Completed and signed application form.
2. Official transcripts of all college work.
3. A minimum GPA of 3.0 on a 4.0 scale.
4. Three letters of recommendation.
5. A 1000 word letter of intent.
6. A current curriculum vitae.
7. Official report of the Graduate Record Examination (GRE)
taken within the last 5 years with a minimum recommended score
of 550 on each section (verbal and quantitative). All applicants
are required to submit a GRE score.
8. International students, whose native language is not English,
are required to submit a paper score of at least 550 or 213
computer score on the Test of English as a Foreign Language
(TOEFL) examination.
9. All PhD students are required to have supporting evidence
of computer literacy, including the ability to use a spreadsheet
program prior to beginning the Ph.D. program.
10. A non-refundable application fee.
11. An interview. Highly qualified candidates for admission
will be contacted for an interview with the program director,
Admissions Committee members and additional faculty from the
Department of Epidemiology and Public Health.
12. Supporting evidence of one course in epidemiology and
two courses in biostatistics.
Each application for acceptance into the Ph.D. program is
examined by members of the Admissions Committee. Materials
submitted in support of an application are confidential and
cannot be released for other purposes nor be returned to the
applicant.
